Baptismal Certificate Request

  • For individuals baptized at Holy Trinity, please fill out this Request Form

  • For individuals baptized at another church. You need to contact that church and request a “newly issued Baptismal Certificate with sacramental notations”.
    You can print, fill out, and send this form to that church:

  • For those baptized in the Archdiocese for the Military Services, you must request a copy of the baptismal certificate from the Office of Sacramental Records at (202) 719-3600 or at: https://www.milarch.org/sacramental-records/

Sponsor/Godparent Form

To be a sponsor/godparent for a baptism or confirmation, you need to:

  1. Print and fill out this form.

  2. Take this form to the church where you are registered and active and request the pastor’s signature and church seal confirming you meet the requirements to be a sponsor/godparent.

  3. Bring the form to the person requesting this form at Holy Trinity.

Solicitud de certificado de bautismo

  • Las personas bautizadas en Holy Trinity deben diligenciar este formulario de solicitud.

  • Las personas bautizadas en otra iglesia deben ponerse en contacto con dicha iglesia y solicitar un “certificado de bautismo recién expedido con anotaciones sacramentales”. Puede imprimir, diligenciar y enviar este formulario a dicha iglesia:

  • Las personas bautizadas en la Arquidiócesis para los Servicios Militares deben solicitar una copia del certificado de bautismo a la Oficina de Registros Sacramentales llamando al (202) 719-3600 o escribiendo a: https://www.milarch.org/sacramental-records/

Formulario de padrino/madrina

Para ser padrino/madrina de un bautismo o confirmación, usted debe:

  1. Imprimir y diligenciar este formulario.

  2. Llevar el formulario a la iglesia en la que está registrado y es miembro activo y solicitar la firma del párroco y el sello de la iglesia que confirmen que cumple los requisitos para ser padrino/madrina.

  3. Entregar el formulario a la persona que lo solicita en Holy Trinity.
